PRIVACY POLICY

Thank you for choosing us as your health care provider. We are committed to your treatment being successful. This letter is to inform you of our new insurance filing procedures. In the past, we have submitted all insurance claims for you and we will continue to do so. Since there has been a high increase with members changing health insurance companies, as well as contract identification numbers, we will request that you have your insurance card present at every visit to verify current information. It will be your responsibility to notify us of any insurance changes. Due to the overwhelming increase in administrative tasks associated with insurance filing we regret that if proper insurance information is not provided to us, it will be your responsibility to resubmit your claim with the proper information and payment will be expected. Also effective immediately, we will no longer be submitting secondary insurance coverage. However, we will provide you with all the necessary paperwork required for claim submission.

Please keep in mind, we do participate with several but not all insurances. If we participate with your insurance company, we will maximize your insurance benefits to the fullest extent. Fee's will be gone over during your initial consultation with the doctor.

Our practice is committed to providing the best treatment for our patients and will chard what is usual and customary within our limits. Please be assured that our office is also committed to serve you and aid in any way possible to maximize your insurance benefits. We remain available to answer all of your questions and again, will continue to submit all primary and secondary insurance claims.
